The name of the limited liability company must contain the words "Limited Liability Company", "LLC" or "L.L.C." The LLC name can not contain language stating or implying that the limited liability company is connected with a state or federal government agency or a corporation or other entity chartered under the laws of the United States.
You will need to provide the name of your LLC, the street addrss of the LLC's principal office, the mailing address of the LLC (if different from the street address), the name of the Registered Agent, and the street address of the Registered Office, the business purpose of the LLC, and the name and address of each of the LLC's member(s).
